As part of the SNAP-Ed statewide support, MFF provides survey design and scanning services. If you are a SNAP-Ed grantee and intend to use these services for your SNAP-Ed program, please review the SNAP-Ed Form Design/Scanning Services Assurances. Agencies using MFF form design and/or scanning services must submit a signed copy of the Assurances to their MFF SNAP-Ed Project Manager. You do not have to submit the Assurances for your statewide evaluation surveys. For more information, contact your MFF SNAP-Ed Project Manager.
