The Food Questionnaire for Adults assesses recent intake of fruit, vegetables, and sugar-sweetened beverages (6 items, pre and post) and health-related quality of life (4 items, post only). Questions were adapted from the Expanded Food and Nutrition Education Program Food & Physical Activity Questionnaire and the Centers for Disease Control and Prevention Health-Related Quality of Life Measures and have been validated in a wide range of populations.
