Submitting Your Year-End Report

We encourage you to read all of the content below to be ready to complete your final report. To help you, MFF will provide resources and technical assistance. Click here to learn more.

 

The FY 2017 year-end report — required — is due to MFF by 4 p.m. on Friday, November 3rd, 2017.

 

The year-end report consists of three parts:

  1. Year-End Program and Evaluation Final Report Narrative that will be submitted as a word document through the MNN website;
  2. Education and Administrative Reporting System (EARS) Data Report that will be completed online in Formstack;

  3. Financial Report that will be completed online in Formstack.

Submission Process

Submit all three components of your year-end report electronically, as well as one hard copy of your year-end report narrative and accompanying appendices.

Electronically

  1. Year-End Program and Evaluation Final Report: in Word (.doc) format with other relevant and supporting materials (appendices) labeled alphabetically in Word or PDF (.pdf) format.As part of your appendices, you are required to submit an Equipment Inventory
    • An Equipment Inventory Template in Excel is available for your use and must be included with your year-end report package. If your organization has its own inventory document, you may use it as long as all of the information in the MNN template is included in your form.
    • MFF defines non-capital equipment as “anything that plugs in” or “anything that recharges by plugging in” that cost less than $5,000 per piece.
  2. Education and Administrative Reporting System (EARS) Data Report: Clicking the ‘Submit Form’ button in the Formstack report will submit this portion of your year-end report to MFF. You will receive email confirmation of your submission.
  3. Financial Report: Clicking the ‘Submit Form’ button in the Formstack report will submit this portion of your year-end report to MFF. You will receive email confirmation of your submission.

Packaging your narrative report and appendices into a zip file groups them together into a single file to make uploading easier. You do not need any special software to create a zip file. If you are not familiar with zip files, please see the instructions provided below.

IMPORTANT: When creating your zip file, please include your organization’s name as part of the file name. For example, if your organization is the “Springfield Association,” you might name your zip file springfield-year-end-report-2017.zip

 

All Electronic files must be received by 4 p.m. on Friday, November 3rd, 2017, following the above instructions.

 

Printed Copy

One complete printed copy of your Year-End Program and Evaluation Final Report Narrative, including appendices, should be submitted to Sarah Panken at the address below, postmarked by 4 p.m. on Friday, November 3rd, 2017.

  1. Your SNAP-Ed program’s final project summary (check with your SNAP-Ed Project Manager to ensure you have the correct version);
  2. Fourth-quarter financial report for current fiscal year;
  3. Participant data files for EARS;
  4. Formstack links for EARS and financial reports; and
  5. Instruction sheets for EARS and financial reporting sections available below.

**These webinars are required for all SNAP-Ed Program Leads**

After you have viewed both webinars, return to this page to fill out and submit the required paperwork confirming your completion. Please submit the EARS and Financial Report Webinar Confirmation Form to your MFF SNAP-Ed Project Manager prior to submission of your year-end report. Note: Reviewing both webinars is a prerequisite for attending TA sessions (see below).

Technical Assistance Sessions

MFF invites Partners to participate in upcoming technical assistance sessions for the FY2017 year-end report. Reviewing the EARS and financial report webinars is required prior to registering for technical assistance sessions.

To assist you during the year-end report process and provide guidance, MFF will offer two technical assistance webinars on:

  • Thursday, September 28th1:00pm – 2:00pm
  • Wednesday, October 4th 10:00am – 11:00am

These webinars are an opportunity for you to ask questions about the SNAP-Ed FY2017 year-end report writing process. MFF staff will not be making a presentation; we will be responding to questions asked at the session. Participants may join the session at any time, but registration is required.

Participation in the TA sessions is not mandatory.

How to Create Zip Files

For Windows users:

  1. Locate the items that you want to zip.
  2. Right click on the file, folder, or files you want to zip
  3. Select “Send to”, and then select “Compressed (zipped) folder”.
  4. Find the newly created .zip archive in the same directory.

For Mac users:

  1. Locate the items that you want to zip.
  2. Right-click on a file, folder, or files you want to zip.
  3. Select “Compress Items”.
  4. Find the newly created .zip archive in the same directory.
